Whenever you enter combat keep up your three backbar buffs, vigor, quick cloak, and hardened armor, in that priority. Prioritize staying near your opponents and applying burning embers on as many people as you can for insane passive healing. 

When attacking an opponent, first apply blood craze, swap to fb, stun them for minor brutality and apply elemental susceptibility. Then breath, and reapply embers if it's dropped. You will leap people generally for nightblades or really weak kills, typically execute will be done with a fully charged whip and a shattering rocks CC, which you should be using every 10-20 seconds to the passive healing and to maintain the buff. 

I don't really know how good serpents disdain is, if it's piss you can always use new moons, wretched vitality etc until you can get your hands on a better setup, such as vateshran fb with bloodspawn monster set and 2 pieces potentates, or 1 druid 1 trainee jewelry.
