If you'd like more crit chance, as I haven't built to that, replace hammers with daggers.
Pale order ring can be replaced if you feel you don't need the extra healing. Just replace with macabre vintage ring and replace helm with slime claw medium or a set of your choice. Or you can switch for a ring of Macabre Vintage and run Velothi Ur amulet.
Brutality and sorcery on the Soul burst skill could be swapped out seeing as we already have that covered by crit surge which needs to be kept up at all times anyway.
I run soul harvest on the front bar ultimate mainly for that spike in damage it gives me when I need it, you can easily swap that out if you like for meteor variants or anything else available to us. Flawless dawn breaker on back bar for fighters guild passives mainly. If you run the bear, it'll disappear if you don't have it on both bars, personally I prefer the passives I get to the summon, but you can tailor it to whatever you want.
I usually start by crit surge (keep up at all times). Hail, caltrops, soul burst then quickly poison injection and swarm on biggest enemy, finish with whirling or killers blade.
Crit surge is used instead of the netch from animal companions for the extra healing when we do crit and because we get sorcerer passives which are advantageous to the build overall. It could be possible to drop the sorcerer subclass and switch out for something else, but to be honest I've not really looked that far into it yet.
